What is an Aging Life Care Manager?
An Aging Life Care Manager also known as a Geriatric Care Manager (GCM) is a consultant who specializes in assisting seniors, and their families who care for them. Our experience and extensive knowledge offer you practical, affordable, and time-sensitive solutions.
How do I know if our family would benefit from the help of a care manager?
You may want to ask yourself…
- Is meeting these needs hindering my daily life and/or my normal responsibilities?
- Is my family or my work being affected?
- Am I having difficulty monitoring services or resources that I have chosen?
- Are my finances being affected by lost work hours?
- Am I spending excessive time and money on travel requirements due to distance from a loved one?
- Do I have the time, skills, resources, and knowledge to engage the proper professionals?
- Are we providing the best care for our aging spouse, parent, or grandparent?
- Why are we paying two service providers for some overlapping services? Can we reduce to save money?
What can Senior Support Solutions do for our family? How do these services work?
Each family is different but we rarely meet a family who cannot use some level of our assistance. Whether they just need us to answer a few questions or provide them with some basic guidance, or they need some crisis management, or they are in a situation where the aging loved one is struggling and the family does not know the best way to help in their care, there is a place to utilize our experience.
The Steps
- First we meet for a free consultation to determine at what level a Care Manager would benefit you.
- After determining the benefits, we will assess the client and their current living situation (home, hospital, assisted living, etc.) with the client, family members, and involved parties. This assessment will review current and needed systems, guiding care plan options to help the family caregiver make informed decisions.
- We assist the client and their family in implementing the plan by connecting them with professional service providers (e.g., physicians, therapists, legal counsel, in-home care). We continue as private consultants overseeing and monitoring the client’s ongoing care.
- Families also ask us to visit to monitor care being provided even when our client lives in a community that provides some care services such as an assisted living facility or memory care facility.
- By providing consistent oversight and monitoring, continuity of care and the best outcome for the client’s well-being is established. We can see how the care plan is working, communicate with family about the success or concerns surrounding care, and most importantly, we can see if there are new issues that family needs to address immediately.
- We provide ongoing communication between the client, designated family members, medical personnel, service providers, and other professionals.
- Additional things we are often involved with include crisis intervention (meeting them at the emergency room), assistance with moving, planning and attending personal celebrations, and getting out to socialize or enjoy their hobbies or other engagement events they want to attend.
- We are advocates for the client and their family, providing support, education, guidance, with a focus to ensure the quality of care that your loved one deserves.
- We also specialize in monitoring expenses being charged by service providers or their living community, as there are sometimes questions concerning fees.
This is a brief list of the things we help monitor. We often have solo agers reach out to us to help them plan ahead and make sure all of their wishes are documented and safeguards are in place so that their care is properly monitored when they need assistance but don’t have family or friends that can help make sure their care is the quality they want.
Does insurance pay for Care Management services?
Unfortunately, most insurance plans have not yet recognized the value and tremendous savings that Care Managers offer. However, some Long Term Care insurance plans do offer reimbursement for services provided.
Is hiring an Aging Life Care Manager something I can afford?
That is a personal decision for each family to make.
- What many don’t realize is that there can be substantial cost savings from hiring a Care Manager because we review your services and often find that money is being wasted on unnecessary services or there are community options that families did not know about.
- We also make sure that care is provided properly the first time so money spent on long-distance travel is avoided and lost time and revenue from missed work is greatly reduced. More importantly, the health and well-being of your loved one, is preserved, which may reduce hospitalizations and medical expenses.
- And, we become even more affordable when we help avoid inappropriate housing placement, uninformed decision-making, and work to help your loved one age in place in their own home as long as possible.
Will I or my loved one lose my independence if a Care Manager is assisting us?
It’s actually just the opposite. You or your loved one will often regain independence that may have been lost, depending on current well-being and health. Your Care Manager works with you, as a partner, to advise, advocate, educate, and support all care needs to encourage independence, wellness, and improved quality of life.
