Geriatric care managers (GCM) can ease the burden of one trying to make significant decisions with little or no background or experience in this area. The education, expertise, and experience of a GCM can ease this transition by creating a proposed Plan of Care including recommendations about the care that is needed. The GCM also oversees, coordinates, and monitors the outcome of the recommendations as well as any resources that have been brought into the picture. As well, A GCM advocates for the senior and their family.

Attend Care Conferences
We manage and participate in ongoing communication and meetings between the client, designated family members, medical personnel, service providers, and other professionals.
Moving Assistance
Should it be necessary for your loved one to move from their current place of residence, we will help provide the necessary outside resources to ensure a smooth transition.
